• Unknown if a 6″ octave keyboard (DS6.0) reduces injury risk for small hand pianists. • Posterior forearm activation (HD-EMG) and finger kinematics were recorded. • Small (vs large) pianists had greater forearm activation on the standard keyboard. • Small pianists also exhibited finger postures associated with higher injury risk. • The DS6.0 reduced small pianist forearm activation and improved finger postures.
