• Provides a critical assessment of lab-on-a-chip (LOC) platforms that integrate optical nanobiosensors for use in point-of-care (POC) diagnostics and clinical decision support. • Overviews recent advances in microfluidic LOC platforms integrated with nanobiosensors for on-chip target detection via optical readouts (colorimetric, fluorescence, chemiluminescence, SPR, SERS, and smartphone-based), enabling rapid, highly sensitive sample-to-result POC testing. • Highlights Recent advances in smartphone-enabled, AI-supported nanobiosensor-integrated LOC platforms for improve analytical sensitivity and specificity, as well as the quality of rapid POC disease diagnostics and clinical decision support. • Addresses key translational challenges, including experimental reproducibility, methodological, operational scalability, integration and implementation within existing regulatory frameworks. • It outlines future directions in device miniaturization, scalable nanofabrication, and clinically translatable POC platforms while highlighting current knowledge gaps, key research priorities, and emerging opportunities for advancing next-generation diagnostic technologies.
