• ADCs bypass, not reverse, multidrug resistance—succeeding where inhibitors failed. • Cleavable linkers drive bystander killing that overcomes tumor antigen heterogeneity. • T-DXd benefit extends from HER2-ultralow through HER2-positive disease. • ABCB1/ABCG2 polymorphisms govern ADC payload retention, efficacy, and toxicity. • Pooled T-DXd PFS HR 0.47 (HER2 continuum); 11.4% ILD across deruxtecan-payload ADCs.
