Antimicrobial susceptibility pattern of selected diarrheagenic pathogens isolated from children under five years, Southwest Ethiopia, June - August, 2023

Diarrheal diseases are a common disease worldwide in children under five years of age, especially in developing countries. Enteric bacteria are the most common causative agents of diarrhea and have become increasingly challenging to prevent and treat due to the emergence of antimicrobial resistance. This study aimed to determine the antimicrobial susceptibility Pattern of selected diarrheagenic pathogens isolated from children under five years, Southwest Ethiopia, June - August, 2023’. A cross-sectional study was conducted from June to August 2023 on 382 children under five years of age in Southwest Ethiopia. A questionnaire was used to collect data from the parents/guardians of each child. A convenience sample technique was used to collect the data. Each collected stool sample was inoculated onto Selenite F broth, MacConkey agar, and Xylose-Lysine Deoxycholate (XLD) agar media. Antimicrobial susceptibility testing was performed using the Kirby-Bauer disk diffusion method on Muller Hinton agar. Socio-demographic, clinical, and laboratory results data were entered in EpiData v4.6.0.6 and analyzed using SPSS version 25. Descriptive summaries were used to describe the study variables. From the 382 children screened, bacteria and parasites were isolated from 50 (13.1%). This included: 17 (4.5%) Escherichia coli, 14 (3.66%) Salmonella species, 11 (2.9%) Shigella species, six (1.6%) Giardia lamblia, and two (0.5%) Entamoeba histolytica/dispara. The highest resistance among bacteria was observed against ampicillin (88.2%), and co-trimoxazole (76.5%). Almost all of E.coli, were susceptible to imipenem, cefepime, and cefotaxime. Of the total 42 isolates, 11 (26.2%) showed multi-drug resistance. Overall, E. coli, Salmonella species, Shigella species, and protozoan parasites are the isolated pathogens from stool samples collected from children under five years of age. Enteropathogenic E. coli was the most common strain group causing diarrhea in the study area, followed by enterohemorrhagic E. coli. The isolated E. coli were susceptible to imipenem, cefepime, and cefotaxime. For all identified bacteria, highest rates of antimicrobial resistance were observed for ampicillin, and co-trimoxazole. The prevalence of multiple antimicrobial resistances was high among these bacteria. It is better to use effective antimicrobials such as imipenem, cefepime, and cefotaxime as treatment options.
