• AI feedback is equivalent or superior to expert human instruction in some procedural simulation settings. • The field is immature: 67% of included studies presented a moderate risk of bias. • Identified the "hidden curriculum": AI optimization leads to unintended skill trade-offs. • Explainable AI (XAI) is critical to address algorithmic bias and build trainee trust. • Future research must prioritize long-term skill retention and human-AI collaboration.
