• Dynamic pupillometry can objectively and quantitatively evaluate diabetic autonomic neuropathy. • Both optical coherence tomography and optical coherence tomography angiography angiography are noninvasive techniques detecting subtle microvascular and neurodegenerative changes of retina. • Static pupillary abnormalities appear before clinical diabetic retinopathy, while dynamic reductions coincide with its onset. • Pupillary abnormalities correlate with retinal neural layer thinning and reduced vascular density in diabetics. • Evaluation for autonomic nervous dysfunction is recommended for all patients with diabetic retinopathy.
