• 100 systematic review protocols had low adherence to some PRISMA-P 2015 items. • Six items were often not reported, 11 items were often only partially reported. • 15 interviewed protocol authors suggested modifications to PRISMA-P 2015. • Findings will provide context for users and inform the update of the reporting guideline.
