Familial partial lipodystrophy (FPLD) is a rare disorder of selective adipose loss associated with extreme insulin resistance, severe hypertriglyceridemia, and metabolic dysfunction associated steatotic liver disease (MASLD). Management is often limited by medication intolerance and, in patients with recurrent pancreatitis, avoidance of incretin-based therapies. Automated insulin delivery (AID) systems offer significant potential to simplify complex insulin regimens, however, clinical data are sparse regarding application in rare lipodystrophy syndromes where insulin requirements often exceed standard pump delivery limits. We describe use of an AID in a patient with FPLD, refractory hyperglycemia, and recurrent pancreatitis.
