• Whole-pelvic radiotherapy (WPRT) remains a debated strategy in localized prostate cancer, particularly for intermediate-risk patients. • In this multicenter retrospective study including 300 patients with intermediate-risk prostate adenocarcinoma, WPRT was not associated with improved relapse-free survival or overall survival. • Rates of acute and late genitourinary (GU), gastrointestinal (GI) and sexual toxicities were similar between groups, but late grade ≥ 3 GI toxicities were significantly higher in the WPRT arm. • These findings should be interpreted with caution given the retrospective design, potential selection biases, and limited statistical power. • Prospective randomized trials and novel predictive tools, such as multi-omic and AI-based approaches, are warranted to better define which subgroups may benefit from prophylactic pelvic nodal irradiation.
