• Blumer's shelf reflects disease in the pouch of Douglas, the caudalmost part of the peritoneal cavity.
• It can be detected by rectal examination as an extrinsic anterior mass just above the prostate/vaginal vault.
• Though best known from peritoneal metastases in cancer, a rectal shelf can occur from diverse non-cancerous causes.
• Among responsible cancers, many other primary sites have been reported.
• Non-malignant causes include endometriosis and active or healed abscesses from in or about the peritoneal space.
• Just as in Blumer's 1909 report, a key implication for today is the indispensability of rectal examination in every case of obscure abdominal disease.
