• Patients with β-thalassemia major (β-TM) often exhibit cognitive impairment, but the underlying brain microstructural alterations remain unclear and lack objective neuroimaging evidence. • Quantitative SyMRI revealed both decreased and increased T1 values in gray and white matter regions, which correlated with cognitive decline and hematological abnormalities in β-TM. • This study highlights the potential of SyMRI as a non-invasive imaging tool for early detection of cognitive dysfunction in β-TM, aiding timely clinical intervention.
