• HER2-negative breast cancer induces systemic remodeling of monocyte subsets. • CCR2+ non-classical monocytes increase four-fold in early-stage patients. • The %CCR2+ non-classical subset shows 96% diagnostic sensitivity and specificity. • High diagnostic accuracy is maintained in early disease stages (I–IIB). • CCR2 expression is independent of systemic CCL2 and inflammatory markers.
