• This review summarizes candidate molecular determinants of Macklin effect risk. • ARDS-associated genetic variants may contribute to alveolar fragility. • TIMP1, MMP8, PTX3, and CCL2 were recurrently identified across omics studies. • Multi-omics data support molecular stratification of ARDS subphenotypes. • Dedicated prospective studies are required to validate these candidate pathways.
