• Continual learning enables AI to accumulate pace-mapping knowledge across patients. • A neural surrogate learns pacing-site-to-electrocardiogram mappings without setup. • Active learning reduces the pacing sites needed to localize ventricular tachycardia. • Knowledge transfer works across heart geometries and different tissue conditions. • The proposed method localizes within 5 mm in 82% of cases using 4.5 pacing sites.
