• CD36 is strongly upregulated in plasma and adipocytes of obese children. • CD36 promotes adipogenesis via PPARγ and suppresses PI3K/AKT signaling, driving insulin resistance. • CD36 directly binds ANXA1 to activate the TLR4/NF-κB inflammatory pathway. • Blocking TLR4 reverses CD36-induced cytokine release and lipid accumulation. • The CD36/ANXA1/TLR4/NF-κB axis represents a potential therapeutic target in childhood obesity.
