• Farmyard connected drainage ditches are “high risk loss pathways” for farm N and P. • Grab water samples showed temporal and spatial variability of concentrations. • Local hydrological connectivity and ditch configuration were influencing factors. • Bespoke wooden leaky dams were installed to slow flow and attenuate nutrients. • Downstream nutrients were reduced and P-rich sediment accumulated behind dams.
