• NST reduced surgical burden in 63 % of patients with locally advanced OCSCC. • Surgical de-escalation was strongly associated with pathologic response. • Negative margins were maintained in 97 % of de-escalated cases. • Reduced resection and reconstruction were the main drivers of de-escalation. • Surgical burden should be considered as an endpoint for neoadjuvant oral cancer trials.
