• IGF-1 levels show a U-shaped association with CAD severity. • IGF-1 levels below the median are linearly associated with adverse outcomes. • Hypovitaminosis D and low IGF-1 together increase residual cardiovascular risk. • IGF-1 and vitamin D dysregulation may impair vascular recovery post-AMI.
