• Trans 3-methylglutaconyl (3MGC) CoA is an intermediate in leucine catabolism. • Trans 3-MGC CoA can also be generated de novo from acetyl CoA. • The molecule is unstable and susceptible to spontaneous chemical reactions. • Organic acids, acylated proteins and acylcarnitines are byproducts of trans-3MGC CoA. • 3-methylglutaryl (3MG) carnitine has been correlated with acute and chronic disease.
