• Limited data guide (neo)adjuvant chemotherapy in young BRCA carriers • This is the largest study including young BRCA1/2 carriers with a prior history of BC • No DFS or OS differences were observed with adding taxanes to anthracyclines • Platinum use in TNBC was not associated with different survival outcomes • Our findings may inform future prospective de-escalation studies
