Clinical Efficacy and Safety of Photodynamic Therapy for Treating Rosacea: A Systematic Review and Meta-Analysis

To assess the efficacy and safety of photodynamic therapy (PDT) for the treating rosacea, seven databases were systematically searched for relevant studies from 1963 to 2025. At last, 23 studies involving 1439 participants were included. In the 17 randomized controlled trials (RCTs) included, PDT all demonstrated considerable efficacy without obvious adverse reactions. As aminolevulinic acid (ALA)–PDT was reported in most RCTs, it underwent a meta-analysis. In eight RCTs, ALA–PDT had far higher efficacy (RR 1.404, 95% CI 1.12–1.762) and cure rate (RR 1.447, 95% CI 1.028–2.038) than the control and far lower papule scores than the control (SMD -2.204, 95% CI -3.547 to −0.861). The ALA–PDT and control groups had no significant difference in the incidence of redness and swelling, burning sensation, blisters, pigmentation, and adverse reactions, displaying comparable safety profiles. In the remaining six single-arm studies included, erythema had the highest incidence (79.7%), while the incidence of mild exudation and mild pigmentation was as low as 21.8% and 19.9%, respectively. No publication bias was observed by Begg’s test and Egger’s test. PDT can effectively alleviate the condition of patients with rosacea, and it is comparable with traditional treatments in the incidence of adverse reactions.
