• Sleep loss induces gut dysbiosis and systemic inflammation. • Gut microbiota modulates memory and attention via neuroactive metabolites. • Obstructive sleep apnea is linked to reduced microbial diversity and pro-inflammatory species. • Probiotics, prebiotics, and melatonin may restore microbiota and improve cognition. • Longitudinal studies are needed to validate sleep–gut–cognition interactions.
