• DEJ microarchitecture places collagen XVIIα1 at the core of epidermal resilience. • Collagen XVIIα1 proteolysis links oxidative and mechanical stress to stem cell exhaustion. • Hair and skin aging converge on a collagen XVIIα1 dependent niche imbalance. • Translational avenues include protease blockade, rhCOL17 and delivery platforms. • Open questions include membrane incorporation, dosing, PK and clinical endpoints.
