• Nanodiamond (ND) with upconversion nanoparticles (UCNPs) was proposed for HepG2 liver cancer treatment. • NDs enable a high local concentration of DOX, and UCNPs trigger the blue light for boosting ROS generation. • In vitro and in vivo examinations indicate significant anticancer effects from combined chemotherapy and photodynamic therapy. • No observable toxicity or tissue damage in major organs was found, supporting clinical safety.
