• The increasing use of microaxial flow pumps warrants a standardized management to maximize the device efficacy and minimize associated complications. Cardiovascular imaging plays an essential role throughout the entire support duration, but guidance on how to integrate it into acute patient care is scarce. • Multimodality imaging for patients receiving circulatory support with microaxial flow pumps comprises a wide array of techniques (ultrasound, fluoroscopy, computed tomography, and nuclear imaging) that can be used to guide patient selection, device positioning, and gather essential information on native heart function and hemodynamics during support. • Future imaging studies on microaxial flow pumps should focus on how to refine the assessment of device positioning and improve the accuracy in predicting heart recovery.
