• 8-epidiosbulbin E acetate is a key toxic component of Dioscorea bulbifera L. and causes significant liver damage. • Cytochrome P450 2C19 is the primary enzyme that efficiently drives the metabolic activation of 8-epidiosbulbin E acetate. • 8-epidiosbulbin E acetate is taken up by the liver via organic anion transporter 3 and expelled via P-glycoprotein. • Inhibiting cytochrome P450 2C19 or organic anion transporter 3 reduces liver damage caused by 8-epidiosbulbin E acetate and Dioscorea bulbifera L.
