• Developed a novel prognostic model based on phospholipid metabolism-related genes for osteosarcoma. • Identified a seven-gene signature (GM2A, PIP5K1C, VAC14, PLEKHA8, UBE2L3, RSPO3, CEBPA) with high predictive accuracy. • The risk model effectively stratifies patients into distinct prognostic subgroups with differential immune infiltration. • VAC14 is a potential therapeutic target, with Vorinostat shown to upregulate its expression and induce apoptosis. • Revealed a link between phospholipid metabolism and the TME, suggesting a metabolic-immune axis in OS.
