• Ex vivo glucose-stimulated insulin secretion shows marked inter-donor heterogeneity in isolated human islets. • A direct correspondence between ex vivo islet function and in vivo β cell function emerges selectively in T2D. • Ex vivo insulin secretion is positively associated with glucose-driven β cell function and inversely associated with potentiation in T2D. • In T2D, impairment of network-dependent regulation may unmask intrinsic islet defects and enable direct in vivo – ex vivo functional mirroring.
