• First systematic review of CPAP and cardiovascular risk in nonsleepy OSA. • Pooled RCT data show a modest diastolic blood pressure reduction with CPAP. • No significant pooled systolic blood pressure reduction was observed. • Cardiovascular event findings were inconsistent, often adherence-dependent. • Standardized endpoints and longer follow-up are needed in future trials.
