• Dream alterations were reported by 83% of patients in suicidal crisis. • Nightmares appeared months before crisis, while suicidal dreams emerged days before. • Over half of patients showed successive changes in dream content before consultation. • Nightmare severity was associated with insomnia severity and anxiety symptoms. • Dream assessments may help detect imminent suicide risk in psychiatric emergencies.
