• Nationwide claims data of 1,439 durvalumab patients after CRT for NSCLC. • Median OS was 48.4 months despite a median age of 73.6 years. • 12-month pneumonitis incidence requiring steroids was 19.4%. • IMRT use was associated with reduced pneumonitis risk (sHR 0.74). • Durvalumab rechallenge after pneumonitis was given in 37.5%.
