• Gastric emptying after PEG preparation is rapid; 68.7% of patients reach a low-risk state at 2 h. • Diabetes is a significant independent predictor of delayed gastric emptying. • Overweight BMI (25.0–29.9 kg/m2) is unexpectedly associated with faster gastric emptying. • A 2-h fast is feasible; gastric ultrasound aids individualized risk assessment in high-risk patients.
