• Susceptibility to Ferroptosis: Rotenone triggers damage in MELAS fibroblasts, including elevated ROS, lipid peroxidation, and Fe2+—heteroplasmy-dependent. • MELASHi is more vulnerable: Baseline Fe2+↑ and GPX4/xCT↓ compared to MELASLow. • DFO (1–100 μM) rescues: Restores cell survival rate, curbs mitochondrial fragmentation.
