• EEG variability is a finding observed predominantly in autoimmune encephalitis. • The absence of seizures and EEG changes had a high negative predictive value for ruling out acute inflammatory encephalopathy. • A variability score calculated from repeated EEGs summarizes temporal and spatial changes, with a higher score (>2) suggesting AE.
