• ECT response and symptom reduction were higher in research cohorts than real-world cohorts before adjustment. • These differences were largely driven by patient selection, comorbidity, and treatment history. • After adjustment, ECT effectiveness was comparable across settings. • Findings highlight that ECT is robustly effective across diverse clinical setting and complex real-world patient profiles.
