• Analyzed 7362 pT1N0 TNBC patients using a nationwide real-world database. • Investigated the clinical efficacy of adjuvant chemotherapy in TNBC patients. • Adjuvant chemotherapy significantly improved survival for T1c (1.1–2.0 cm) tumors. • No survival benefit was observed for tumors ≤ 1.0 cm (T1a/b).
