• Adjunct anti-microbial-photodynamic-therapy (aPDT) provided only short-term periodontal benefits in patients undergoing clear-aligner-therapy (CAT). • No added benefit of aPDT over mechanical debridement alone was observed in fixed orthodontic therapy (OT) group. • Periodontal improvements in CAT were not sustained at 3 months’ follow-up. • Longer fixed OT duration correlated with worse periodontal parameters. • Oral hygiene compliance influenced periodontal outcomes in both groups.
