• This network meta-analysis compared 14 pharmacological regimens in 8186 patients with RA-ILD from 27 studies. • Methotrexate, tocilizumab, and rituximab were associated with lower all-cause mortality, with ORs of 0.15, 0.28, and 0.33, respectively. • Abatacept plus methotrexate was associated with reduced ILD progression risk, with an OR of 0.07 versus control. • Immunoglobulin was associated with higher FVC%, whereas nintedanib and pirfenidone were associated with slower FVC decline or preservation. • JAK inhibitors showed favorable tolerability, with a 7.9% discontinuation rate and 81.7% drug retention.
