• LLM-generated plain-language portal summaries improved comprehension at Day 14. • Claude Sonnet 4.5 and GPT-5.1 Thinking showed comparable patient outcomes. • Portal engagement and self-management adherence rose in both LLM arms. • Combined LLM arms had fewer return visits and antibiotic requests. • Clinician audit found no clinically significant hallucinations.
