• Peripheral β-endorphin and met-enkephalin levels were reduced in trichotillomania. • Focused hair-pulling subtype showed more pronounced β-endorphin reduction. • Opioid peptide levels were associated with hair-pulling phenomenology. • Endogenous opioids distinguished trichotillomania from healthy controls. • Findings support reward- and stress-related mechanisms in hair-pulling behavior.
