• Acute ethanol exposure prior to burn injury potentiates neuroinflammation and enhances inflammatory gene expression in microglia. • The combined insult of ethanol exposure and burn injury disrupt intestinal and blood–brain barrier integrity, increasing bacterial translocation and brain albumin accumulation. • Dysbiosis of the gut microbiome links peripheral injury to neuroinflammation, highlighting the gut–brain axis as a therapeutic target. • Findings support a mechanistic role of the gut–brain axis in linking ethanol intoxication with worsened neuroinflammatory outcomes after burn injury.
