Exercise may modulate glymphatic function, providing a plausible mechanistic link between physical activity and neuroprotection. Key regulators of glymphatic function overlap with physiological adaptations to exercise, suggesting a mechanistic basis through which physical activity may preserve glymphatic integrity and support brain resilience. Sleep disruption may impair glymphatic function and contribute to cognitive decline, while exercise may help restore sleep health and facilitate brain waste clearance. Future development of clinically validated imaging methods to quantify glymphatic function in humans will be crucial for defining its role in neurodegeneration and evaluating interventions such as exercise.
