• Exercise-based prehabilitation significantly reduced hospital length of stay by nearly 1-day. • Exercise-based prehabilitation reduced the risk of overall postoperative and pulmonary complications in patients with cancer undergoing surgery. • The most effective programs involve supervised, multidisciplinary aerobic or concurrent training. • Larger effects were noted in women and older patients. • This study supports the integration of structured, supervised aerobic or concurrent prehabilitation intro perioperative care pathways.
