• BMI and depression show a sex-specific association in adolescents with MDD. • Females with abnormal BMI exhibit higher depressive severity than normal weight. • Inflammatory markers increase progressively across ascending BMI categories. • Neutrophils and MLR show significant indirect effects in the obesity–depression association. • Targeting immune–metabolic pathways may improve personalized MDD treatment.
