• One in five cancers were diagnosed outside of cancer fast-track pathway. • Fast-track cancer diagnosis was associated with earlier stage at diagnosis. • Compared to fast-track pathway, unplanned hospital admissions had the worst prognosis across majority of cancer types. • Compared to fast-track, planned hospital admissions showed early stage at diagnosis but no survival advantage. • Diagnostic benefit varied by cancer type and diagnostic difficulty, highlightning the need for differentiated system-level strategies.
