• FGF20 alleviates neuroinflammation by modulating microglial M1/M2 polarization. • FGF20 upregulates TREM2 via FGFR1 and inhibits TLR4/NF-κB independently of FGFR1. • FGF20 extends its role to immunomodulation, highlighting therapeutic potential for stroke.
