• CCTA-derived fluid dynamics-informed parameters predict abnormal FFR in R-AAOCA. • Conventional and fluid dynamics-informed parameters were comparable for FFRAdenosine. • Resistance Index adds a physiologically motivated complementary descriptor. • Resting CCTA alone cannot fully capture stress-induced dynamic compression.
