• FOXC1 is a novel transcriptional regulator in atherosclerotic lesions, genetically linked with CAD and SMC-to-macrophage ratios in plaques, which warrants translational studies. • FOXC1 is down-regulated in plaques and intimal hyperplasia and localized to SMA+ cells. • FOXC1 regulates SMC quiescence vs activation by modulating key pathways involved in cell adhesion, cell cycle progression, and actin cytoskeleton organization.
