• Cryo-EM structures illuminate the ‘dual-key’ mechanism that allows TSHR to be activated by hormones and autoantibodies. • Functional antibody subtyping outperforms traditional binding assays for precise management of Graves‘ disease. • Blocking monoclonal antibody K1-70 exhibits thyroid function-suppressing efficacy in Phase I clinical trials. • Dual-target strategies that disrupt TSHR/IGF-1R crosstalk (e.g., teprotumumab) are beneficial in treating Graves’ orbitopathy.
