• ARCP panels are less diverse than UK surgical trainees by gender, ethnicity, and sexual orientation. • Women, ethnic minorities, and LGBTQ+ surgeons had significantly lower odds of ARCP panel membership. • Consultant-only analysis showed reduced disparities, suggesting pipeline effects in progression. • Persistent ethnic underrepresentation highlights barriers extending beyond the trainee level. • Improving assessor diversity is a practical step toward equitable surgical training and leadership.
